Position Summary

Reporting to the Executive Lead-Regional Chronic Diseases, the Lead, Chronic Diseases Strategic Initiatives-Kidney Care & Diabetes provides leadership, coordination, coaching, quality improvement and evaluation support to the regional Kidney Care and Diabetes Care Programs, to the implementation of the NH Kidney Care Action Plan, and to other disease specific work as identified. The Lead works in a co-leadership model with the Medical Lead of the NH Kidney Care Program.

Starting salary will be approximately from $88,990 to $111,237 and will be based on education, training, experience, and salaries of similar positions.

Qualfications
• Master's degree in Nursing, Health Administration, Business Administration or Health related field;
• Five (5) to seven (7) years recent related health care experience including two (2) years in a management role, or an equivalent combination of education, training and experience.
• Additional leadership training an asset.

Skills and Abilities:
• Thorough knowledge of the principles of chronic diseases management.
• Thorough knowledge of a multidisciplinary team approach involved in the coordination of care in a complex environment and system of care services.
• Competence in data analysis including interpretation and the ability to orally report and also represent key findings in writing.
• Strong oral and written communication skills.
• Project management skills demonstrating the ability to transition proposals and plans into operations.
• Proven ability in facilitation and consensus building among multidisciplinary groups.
• Ability to promote team building while understanding that decisive action is required.
• Strong leadership and people management skills with the ability to motivate and delegate.
• Ability to find creative solutions to problems and to develop alternative strategies to support quality improvement initiatives.
• Positive problem solving skills in a dynamic unionized work environment.
• Strong planning, critical thinking and analytical skills.
• Knowledge of kidney disease and diabetes and leading practices in kidney care and diabetes care.
